Discharged patients have online access to their hospital record via the Relay Health Patient Portal.
During registration, patients are given a pamphlet with instructions on how to access the Portal. If they provide a valid email at registration, they will receive an invitation to join the Portal once their visit is discharged. Upon receipt of their e-mail, patients then create an account completing a short set of questions to verify their identify. Only patient visits discharged after June 17, 2014, are available.
The Relay Health Patient Portal is the patient’s electronic Personal Health Record. The patient has the ability to maintain their Personal Health Record by adding or editing their health information, however the information is not part of the hospital information system.
