Medical Nutrition Counseling

Instructions:

  • Please arrive 5-10 minutes prior to your appointment time.
  • Check in at the Education Department (see reverse side for a map)
  • The session will last one hour.
  • Medicare will only cover this service for Diabetes and/or Renal Failure. If Medicare is your primary payer and you do not have one of these diagnoses you may be responsible for payment.

Glucometer Education

Instructions:

  • Please arrive 5 minutes prior to your appointment time.
  • Check in at the Education Department (see reverse side for a map)
  • You will need to bring the glucometer, with package inserts, to your appointment.
  • Bring the note from your physician on how often to check your blood glucose.
  • The session may last one hour.
  • This service is provided at no charge.

Insulin Education

Instructions:

  • Please arrive 5 minutes prior to your appointment time.
  • Check in at the Education Department (see reverse side for a map)
  • Bring your insulin, with package insert, and syringes with you on the day of your appointment.
  • Bring your physician order/script or have your physician fax to 217-479-5875.

New Start Date for Walking for Wellness

Walking for Wellness at the Jacksonville High School Bowl returns for another season of exercise and fellowship Monday, Oct. 2. The Walking for Wellness program provides a safe place in which to get walking exercise when weather conditions make walking outside unpleasant or dangerous. The program is open 7 a.m. to 1 p.m. each day Jacksonville School District 117 is in session. The program is closed during school holidays (including Thanksgiving and Christmas vacation) and any day school is not in session. On average, 75 people walk at the JHS Bowl each day the program is open—resulting in about 6,400 “encounters” with community members during the five months each year that Walking for Wellness is in session. Walking for Wellness is also offered year-round at First Christian Church, 2106 S. Main, in South Jacksonville. The church is open to walkers Monday-Friday 12:30 to 3 p.m. Walkers there should use the south entrance. There is no cost to participate in Walking for Wellness. Registration is not required.
