MyHealth@Memorial Patient Portal

The MyHealth@Memorial patient portal is a secure, easy way to view your medical information as a patient at Passavant Area Hospital. It is a free, online, patient-managed personal health record.

MyHealth@Memorial gives you the ability to view and maintain your personal health records from Memorial Medical Center (MMC), Memorial Physician Services (MPS), Passavant Area Hospital (PAH) and other local providers in one convenient location.  You can download, print and share this information with your provider on your next visit. 

*If you are RelayHealth portal user your account will remain active and accessible, however, as of September 25, 2018, all new Passavant Area Hospital health records will be uploaded to the MyHealth@Memorial website.  You can access your RelayHealth account using the link below. 

How to Sign Up

There are several ways you can sign up for your MyHealth@Memorial patient portal account:

1. Upon Patient Registration
When you are registering as a patient at Passavant Area Hospital, the registrar will ask you if you would like to sign-up for MyHealth@Memorial. If you say yes, we will send an invitation to the email address you provide at registration.

2. After Treatment or Services at Passavant Area Hospital
Bring a valid state or federally issued photo ID and email address to the Health Information Records office at Passavant Area Hospital.

3. Add Passavant as a provider to your existing FollowMyHealth portal account
If you are currently using FollowMyHealth for another local healcare provider, you can simply add Passavant Area Hospital as a new connection.
Log in to your portal account, click on My Account, then click Connections. You will be able to search for and request to connect with Passavant Area Hospital.  To complete the connection, you will need to bring a federally issued photo ID and email address to the Health Information Management Office at Passvant Hospital.

How to Set Up Your Account

1. Receive invitation email from FollowMyHealth.
You will be sent an invitation to the email address you provided at registration or when you presented your ID. The email will come from FollowMyHealth with the subject line of Invitation to join MyHealth@Memorial.

Important Note: If you already have a portal account with MMC, MPS, or any other hospital or clinic, you should not create a new account. Log in to your portal account, click on My Account, then click Connections. You will be able to search for Passvant Area Hospital and request a connection.

2. Click the link in the invitation email
After clicking the link in the email, follow the steps outlined to sign up, or add Passavant Area Hospital to your portal connection.

3. Enter your invitation/security code. 
This code will be your birth year. Entering this code will complete your account set up. 

How to Log In

After your account has been created, go to MyHealthatMemorial.com to sign in or click the Log In Now button below. 

From there, you can use your unique username and password to log in to your MyHealth@Memorial patient portal account. You will always use the MyHealthatMemorial.com site to access your patient portal account.

How to Get Help with Your Account

A user support hotline is staffed Monday-Friday, 7 a.m. – 7 p.m.

Call 888-670-9775 or email: support@followmyhealth.com. Any calls received outside these hours will be returned the next business day.

How to Access Your Relay Health Account

As of September 25, 2018, Passavant Area Hospital is no longer reporting health information to RelayHealth.  Your RelayHealth account will remain open and you can access it here: https://app.relayhealth.com/

 

How to Import Records from Relay Health

If you would like to upload your RelayHealth information to your new MyHealth@Memorial account you can do so by following these instructions:

1. Log in to RelayHealth. 

2. Click on Download Health Data.

3. Change the radial button from XML to PDF and click the Blue Button Download my data button.

4. You will be prompted to save the pdf.  Save the file to a secure location because it is your Personnel Health Record.  Relay Health gives the Adobe file the name of HealthData_Sensitive.  

5. Log in to MyHealth@Memorial and click on the My Health tab. 

6. Click on the Documents Tab.

7. Click on the Add Document button.

8. Name your document and add a date and provider information in the empty fields provided.

9. Click Choose file.  Select the RelayHealth document you downloaded to your desktop.  

10. Press "Save".

Your PDF of RelayHealth information is now stored under the documents tab of your MyHealth@Memorial account.  You can reference it at anytime.
