The Healthy Communities Collaborative, an extension of the Healthy Jacksonville program, connects social service providers who provide access areas where individuals struggle with self-sufficiency.
The Collaborative is divided into four workgroups that address the highest needs identified by clients in the Healthy Jacksonville program:
Mental Illness/Substance Use Disorder
Each group takes on projects to improve access in their respective areas. The Healthy Communities Collaborative will develop tools for case managers to use to assist their clients as they navigate access to key assistance programs.
