Special ServicesContinuity of Care Planner
A Continuity of Care Planner can assist in meeting personal and family needs after discharge, such as:
- Arrange for needed post-hospital care such as nursing, homemaking, and meals
- Obtain medical equipment, such as walkers, wheelchairs, and a hospital bed
- Secure information about community agency services and referrals to those agencies.
A Continuity of Care Planner is available Monday through Saturday to assist you with your discharge needs.
Discharge
Your physician will prepare an order for your discharge. Please notify the nurse of the time you are planning to leave the hospital as she will need time to plan for your discharge.
Prior to leaving, you or a family member should stop by the Business Office to pick up any valuables you may have deposited upon admission.
Before you leave, please be sure that all of your clothing and belongings have been packed. Nursing personnel will assist you in your preparations to leave and will escort you to the main lobby.
Gift Shop
The Gift Shop, sponsored by the Passavant Auxiliary and staffed by volunteers, is just off the main lobby on the ground floor. The Gift Shop offers a variety of items and is a pleasant place to browse. For your convenience, the Gift Shop is open Monday through Saturday from 10 a.m. to 8 p.m. and on Sundays from 1 p.m. to 8 p.m.
Interpreters
Passavant Area Hospital will provide interpreters for hearing impaired or non-English speaking patients whenever communication is necessary for the care of the patient. Interpreters are also provided for community health education programs.
An interpreter will be requested when a hearing impaired or non-English speaking patient or caretaker of a patient presents to the Emergency Department, when an initial evaluation is being done, when a surgical consent is being obtained, when a patient is immediately postoperative, when a patient is in labor and delivery, when an invasive radiology procedure is being done, when decisions regarding resuscitation or advanced life support measures are being discussed, when discharge instructions are being given, and when the family or patient requests an interpreter.
Social Services
A Social Services representative will assist you and your family with practical service needs. At no charge, a representative can assist in meeting personal and family needs before and after discharge, such as:
Complete an Advance Medical Directive
If you want to learn about or complete an Advance Medical Directive, such as Living Will, Power of Attorney for Health Care or Property, a member of our Social Service Department is available to assist. There is no charge for this service.
Arrange Nursing Home Placement
If you or a family member require short-term rehabilitation placement or long-term placement, the Social Services Department is available to assist you and your family.
Administer Memory and Alzheimer Screenings Passavant Area Hospital is a primary provider site for the SIU School of Medicine Memory and Aging Clinic. If you feel you or a family member is having difficulty with memory, contact the Social Services Department for a free Memory/Alzheimer Screening.
Obtain financial information, including Medicare and Medicaid
Social Services will initiate a Medicaid application for eligible persons requiring assistance with medical bills.
Provide confidential patient and family counseling and support
Make referrals to community-based agencies and support groups.
Support Groups
There are a number of support groups that meet monthly at Passavant Hospital. If you would like additional information about one of these groups, such as cancer, multiple sclerosis, coronary, asthma, diabetes, fibromyalgia, lupus, and Alzheimer's, call Educational Services at 217-245-9541, extension 3296, or ask your nurse.
