Rights & ResponsibilitiesThe patient has the right:
- To reasonable access to care;
- To care that is considerate and respectful of his or her personal values and beliefs;
- To be informed about and participate in decisions regarding his or her care;
- To participate in ethical questions that arise in the course of his or her care, including issues of conflict resolution, withholding resuscitative services, forgoing or withdrawal of life-sustaining treatment, and participation in investigational studies or clinical trials;
- To security and personal privacy and confidentiality of information. All communications and records pertaining to his or her care will be treated as confidential by the hospital. Those not directly involved in the patient's care must have the permission of the patient to be present;
- To be informed of the hospital charges for services and available payment methods;
- To know, by name, the physician responsible for coordinating his or her care;
- To designate a decision maker in case the patient is incapable of understanding a proposed treatment or procedure or is unable to communicate his or her wishes regarding care;
- To access protective services, such as advocacy services through the Social Services Department;
- To expect that within its capacity a hospital must make reasonable responses to any reasonable requests he or she may make for service. The hospital must provide evaluation, services and/or referral, as indicated by the urgency of the case;
- To obtain information as to any relationship of the hospital to other healthcare and educational institutions insofar as his or her care is concerned. The patient has the right to obtain information as to the existence of any professional relationships among individuals, by name, who are treating him or her;
- To expect reasonable continuity of care. The patient has the right to expect that the hospital will provide a mechanism whereby he or she is informed by his or her physician, or a delegate of the physician, of the patient's continuing healthcare requirements following discharge;
- To exercise these rights without regard to sex, cultural, economic, educational, or religious background or the source of payment for his or her care.

PATIENT RESPONSIBILITIES:
The care a patient receives depends partially on the patient himself. Therefore, in addition to these rights, a patient has certain responsibilities as well. These responsibilities should be presented to the patient in the spirit of mutual trust and respect.
The patient has the responsibility:
- To provide accurate and complete information concerning his present complaints, past medical history, and other matters relating to his health.
- To make it known whether he clearly comprehends the course of his medical treatment and what is expected of him.
- To follow the treatment plan established by his physician, including the instructions of nurses and other health professionals as they carry out the physician's orders.
- To keep appointments and for notifying the hospital or physician when he is unable to do so.
- For his actions should he refuse treatment or not follow his physician's orders.
- To assure that the financial obligations of his hospital care are fulfilled as promptly as possible.
- To follow hospital policies and procedures.
- To be considerate of the rights of other patients and hospital personnel.
- To be respectful of his personal property and that of other persons in the hospital.
