Tribute Garden 
Passavant Hospital's Tribute Garden is located between the M.R.I. Building and the Rehabilitation Services building behind the hospital's East Wing. The proceeds from the sale of the bricks are used to purchase equipment for the Rehabilitation Services Department and for the upkeep of the Garden.
The triangle of bricks located in the center of the Garden has been replaced with engraved bricks. The bricks can be used as a tribute to a loved one, friend, co-worker, or can be engraved with your name or names of family members. 
The cost of each brick is $100. Payment may be made in full or in four increments of $25 each. Please complete the form and send to the Fund Development Office, Passavant Area Hospital, 1600 West Walnut, Jacksonville, IL 62650.  Each September we have a Memorial Service to honor those whose names are on the Gift Bricks. Family, friends, and co-workers are welcome to attend
